Fundraiser Requests

Please forward any fundraiser requests to Ann Petit at petita@mishawaka.k12.in.us

All fundraisers must be approved by the building Principal and the Superintendent prior to the start date of the fundraiser. A signed copy will be emailed back to the requesting sponsor and the school office.

Please email or call Ann at ex. 4500 with any questions or concerns you may have regarding fundraisers.

This form must be used for ALL fundraiser requests except MPG and MEF

The SA-8 form must be filled out and turned in with your fundraiser deposits

Fundraiser Follow-Up Report

This report may be completed at the conclusion of the fundraiser and sent to the ECA Treasurer.

This form or a spreadsheet should be used whenever students are given items to sell
